Regenerative Orthopedics for Shoulder Pain: Get Back to Pain-Free Movement Without Surgery
The shoulder is a complex joint responsible for a wide range of upper body movement. It consists of bones (clavicle, scapula, humerus), joints, muscles, ligaments, and tendons that work together for stability and motion. Key joints include the glenohumeral joint (connecting the humerus and scapula), acromioclavicular joint (between the scapula and clavicle), and the sternoclavicular joint (clavicle and sternum), all vital for shoulder movement.
Muscles like the deltoid, rotator cuff, and trapezius stabilize the joint and support movement, while ligaments (acromioclavicular, coracoclavicular, and glenohumeral) provide structural support. Tendons of the rotator cuff, bicep, and pectoralis play a crucial role in movement by connecting muscles to bones.

A rotator cuff tear is an injury or degeneration to the group of muscles and tendons that secures the head of the humerus into its shallow socket in the shoulder joint. A rotator cuff tear could result from trauma or overuse of the joint. More commonly, age can begin to degrade the tendon, resulting in a degenerative tear. A degenerative tear can develop from bone spur formation, decreased blood flow, or chronic overuse. There are two types of tears: partial or complete. A partial tear results in the tendon still attached to your humerus, while a complete tear results in complete separation from the bone.
Risk factors for rotator cuff tears:
- Family history
- Smoking
- Age – over the age of 40 years old
- Occupation – mechanics, painters, carpenters, professional athletes
- Poor posture
A labral tear is damage to the cartilage that lines the interior of the ball-and-socket joint, which is the glenoid and humerus. The role of the labrum provides stabilization, which allows for smooth and controlled movement. The cartilage can tear due to repeated overhead or repeated movements., trauma, or degeneration. An acute injury can result in a tear or detachment of the labrum from the socket. There are two different types: SLAP and Bankart tears. A SLAP tear occurs when there is a tear or injury to the superior portion of the labrum, which is the area at the top of the glenoid cavity where the biceps tendon attaches. On the other hand, the Bankart is a tear of the labrum and the inferior glenohumeral ligament from the front (anterior) portion of the glenoid cavity of the scapula.
Risk factors for labrum tears:
- Those who play sports with repeated overhead motion – baseball, golf, tennis
- Those who play contact sports
How to diagnose your shoulder pain?
At Meeting Point Health, Dr. Matta begins shoulder pain treatment with a thorough assessment, including a medical history review, physical exam, and ultrasound diagnostics. He evaluates symptoms, mobility limitations, and any previous injuries. The physical exam includes orthopedic, functional movement, and neurokinetic assessments. Using ultrasound, Dr. Matta examines the internal structures of the shoulder to recommend personalized treatment options for shoulder pain relief.

Prolotherapy therapy is a medical injection procedure whose goal is to relieve pain and promote healing of musculoskeletal conditions, especially involving ligaments, joints, and tendons Prolotherapy stimulates healing, causes inflammatory responses that increase blood flow and the arrival of immune cells and growth factors to the injection area, and tissue repair.
Platelet-rich plasma PRP therapy is a medical treatment that involves the use of a patient’s own blood to promote healing and tissue regeneration. Once injected, the growth factors and other bioactive substances in PRP work to accelerate tissue repair, reduce inflammation, and promote the formation of new blood vessels. This helps in reducing pain, improving function, and enhancing the overall healing process.
Stem cell injection therapy refers to a medical procedure in which stem cells are harvested and then injected into a localized area on a patient’s body, typically to promote tissue repair, and regeneration, or to treat various medical conditions. Stem cells are unique because they can develop into different types of cells in the body, and they also have the capacity for self-renewal. This makes them valuable in medical treatments, as they can potentially replace or repair damaged or diseased tissues. Stem cell injection therapy can treat arthritic joints, sports injuries, tendinopathies, non-union fractures, meniscal tears, Degenerative Disc Disease, and Osteonecrosis or Avascular Necrosis.
Prolozone therapy is a mixture of ozone gas and a prolotherapy solution is injected into the affected area, often around joints, ligaments, or tendons. The theory is that this combination not only irritates and strengthens the tissues (like prolotherapy) but also delivers the potential therapeutic benefits of ozone, such as improved circulation and tissue oxygenation. Prolozone combines procaine, dextrose, B vitamins, saline, oxygen, and ozone into a damaged joint and the combination allows for regrowth of damaged tissue.
Nerve hydrodissection is a minimally invasive medical procedure involving injecting an anti-inflammatory solution around an entrapped nerve. Injectables such as dextrose, vitamins, peptides, plasma, procaine, and even stem cells are flooded around compressed or irritated nerves. This technique aims to separate the affected nerve from surrounding structures, such as muscles, fascia, or scar tissue, which may be contributing to the pain or dysfunction.
Peptide Therapy can be introduced as an at-home treatment. Peptide therapy introduces peptides to signal communication with our cells to perform a specific response. Administrating peptides elicits a response to regulate or improve physiological functions. For regenerative medicine, peptides for wound recovery, and tissue repair, reduce inflammation, accelerate recovery time, and promote cell migration and differentiation.

"Excellent people, very detailed. I'm a USA Boxing Instructor, I have issues with my shoulders from holding the punching mitts, before I walked into their office, I could barely lift my arms, I walked out ready to fight 12 rounds!! Thank you again, very grateful."

"Dr. Matta was the only health care professional able to successfully treat chronic pain in my arm that I had been experiencing for several years. Rothman Orthopaedics incorrectly diagnosed me as having arthritis and had me do physical therapy, but after three months the pain worsened. Dr. Matta figured out it was an issue with my spine and radial nerve. After his treatments, I feel astronomically better. He cared about me wholistically, and is the single best experience with a doctor that I have ever had. He is amazing."

"Dr. Matta has helped me avoid shoulder surgery through stem cell therapy, and I am eternally thankful for that!"
